At New York Life, we understand that the loss of a loved one can be a very difficult and confusing time. That's why we make it our priority to keep the claims process as simple and streamlined as possible.

The quickest way to report a life insurance claim is to complete our online claims form.* Alternately, you can follow the steps below to submit by mail or fax. If you have any questions, or to file a claim by phone, please call us at (800) 695-5165. Our business hours are 8 a.m. to 5 p.m. (Eastern Time).

You can download a claims packet that includes all the instructions and forms needed to begin processing your claim.

To request a claims packet by phone, call New York Life's dedicated claims line at (800) 695-5165. Our business hours are 8 a.m. to 5 p.m. (Eastern Time) Monday through Friday.

Please have the following information available when you call: Contract Number or Social Security Number of insured, Date of Death, and Cause of Death.

2. Complete and Mail or Fax Your Claim Form

Mail or fax your completed claim form and a certified death certificate to:

3. Provide Additional Documentation If Needed

If necessary, a claims representative may contact you to request additional documentation needed to complete your claim. For a complete list, see the Claims Checklist below. These forms, if requested, can be mailed to one of the addresses above.

At any time during the claims process, if you have questions about your claim, please call New York Life's dedicated claims line at (800) 695-5165.

Payments for incontestable claims are usually mailed within 7-10 business days of receipt of a claim form and death certificate.

Claims Checklist

Here is a list of documents you will need to submit in order for us to consider your claim:

1. Completed claim form
2. Certified death certificate (copies not accepted)

The following documents may be requested depending on your specific situation:

3. Proof of guardianship for a minor beneficiary
4. Executorship papers for the estate
5. Funeral Home assignment(s)
6. Proof of accidental death (i.e. newspaper article, police report, etc.)
7. Survivorship Affidavit - Deceased Insured (also available in Spanish)
8. Survivorship Affidavit - Deceased Beneficiary
9. Foreign Death Questionnaire
10. Evidence of Age
